Physical Therapy Services
Here's an overview of our services and link below to book an appointment. We are a cash based business and do not currently accept health insurance. We can provide a super bill upon request. Payment is required at time of scheduling with any balance due upon completion of appointment.
If canceled with less than 24 hours notice, a fee is charged.
Initial Consultation
For in-person or telehealth initial appointments.
$200 for 60 minute consultation.
Follow Up Appointment
For in-person follow up, ongoing appointments in the clinic or telehealth.
$175 for 60 minute follow up.
Telehealth Only
Done via Doxy. Telehealth can be done for initial consultations or follow ups. $200 for 60 minute consultation; $175 for 60 minute follow-ups; or $100 for 30 minute follow-ups.
Traveling to See Expert CCI Provider
Two hour in-person or telehealth consultations are available for travelers for $375 for specialized situations and two hour follow ups for $350.
